Question:

I need your help. My mom has negative record sa NSO but she checked sa munisipyo meron na daw po but her date of birth is totally incorrect but her name and parents’ name are correct. Pwede po bang mag late registration na lang siya since she doesn’t have record in NSO?? Please help me with what to do... Thank you!

Answer:

Filing for Late registration is not the right move.

Hindi allowed ang Late Registration kasi meron talaga siyang record sa munisipyo, hindi lang nai-forward ng munisipyo sa NSO kaya negative ang record niya sa NSO.

Ganito talaga ang process na gagawin, forward muna ang copy ng birth certificate sa NSO, then saka aayusin ang mali, hindi kasi pwedeng mag-bago ng record ang munisipyo kung wala pang record sa NSO.

Question:

My concern is about my gender. Do I need a lawyer if I don’t have a gender identification on my BC? How can I fix it?

Answer:

You can file for Supplemental Report at the Local Civil Registrar without employing a Lawyer.

Hindi mo na kailangan ng lawyer para malagyan ng gender ang NSO copy ng birth certificate mo, Supplemental Report lang ang gagawin dahil lalagyan lang naman ng gender.

Ipasa mo yung NSO Birth Certificate mo na walang gender sa Local Civil Registrar Office (LCRO) kung saan nakarehistro ang birth certificate mo para Makita nila yung kulang na information sa NSO birth certificate mo.

Question:

Hello NSO, Tanong ko lang nangyari kasi ginamit ng kaibigan ko 1989 na year of birth sa lahat ng documents niya SSS, NBI, TIN ngayon pagkuha niya ng BC naka indicate na ang date of birth is 1992 pero ginagamit na talaga niya 1989 eversince. Ano po dapat gawin? Yun kasi yung naka indicate sa baptismal niya.

Answer:

Since the year of your birth need correction, you will need a lawyer to handle this.

Kapag kasi YEAR na ang mali sa NSO birth certificate kailangan mo na ng Lawyer dahil sa korte na ito aayusin, Court order or hearing ang proseso na ito.

Hello po NSO, kumpleto na po ako ng papers para sa endorsement ng birth certificate ko sa NSO ang tanong ko lang po pwede po ba dito ko sa NSO Trece Martirez Cavite magsubmit ng papers sa halip na sa NSO Quezon City, Salamat po

Answer:

Have you processed and filed for an endorsement at your Local Civil Registrar?

Kung tapos na ang process ng Endorsement sa munisipyo kung saan naka-rehistro ang birth certificate mo at meron ka ng FINALITY REPORT, nai-forward mo na rin yung isang copy ng Finality Report sa NSO at nasa iyo na ang RECEIPT ng COURIER at kukuha ka nalang ng CORRECTED COPY ng birth certificate mo sa NSO, YES! Pwede kang kumuha sa NSO Trece Martirez Cavite basta ito ay NSO Office.

Mas matagal nga lang ang release nito, kung sa NSO Quezon City umaabot ng 3 weeks ang 1st copy ng corrected NSO documents, kapag sa ibang branch ng NSO aabot ng mga 1 month or mahigit ang release nito.

Hello NSO, tanong ko lang po kung paano ko mapapalitan ang status ko sa birth certificate ng mga anak ko. Nang ipanganak ko po kasi ang dalawa kong anak ay hindi pa kami kasal ng kanilang ama. Ngayon po na kasal kami, gusto ko po papalitan ang status ko. Ano po ang gagawin ko?

Answer:

You can file for LEGITIMATION sa munisipyo kung saan naka-rehistro ang birth certificate ng mga anak mo.

REQUIREMENTS:

  • Certificate of Marriage;
  • Certificate of Live Birth of the child;
  • Acknowledgement (not required for illegitimate children born on or after 3 August 1988);
  • Affidavit of legitimation executed by both parents which shall contain the following facts:
    1. the names of the parents;
    2. that at the time when child was conceived, the aforesaid parents could have contracted marriage, and that they subsequently contracted marriage,
    3. the date and place when such marriage was solemnized;
    4. the name of the officer who officiated the marriage;
    5. the city or municipality where such marriage was recorded;
    6. the name of the child to be legitimated, and the other facts of birth;
    7. the date and place where the birth of the child was registered; and
    8. the manner by which the child was acknowledged by the parents which may be in the child’s record of birth, in a will, a statement before a court of record, or in any authentic writing (not required for illegitimate children born on or after 3 August 1988).

Hello po NSO.. pwede ba magtanong? Kasi po sa SAUDI ARABIA po ako pinanganak then may birth certificate po ako kaso Arabic po then hindi ko po alam kung paano mapaparehistro ito. Answer:

Have you requested for am NSO copy of your Birth Certificate?

Kumuha ka na ba ng NSO copy ng birth mo sa NSO at negative/intact certificate ang nakuha mo?

Kung wala ka pang rehistro ng birth certificate mo sa NSO, kailangan mo ng mag-file ng late registration, sa case mo na sa ibang bansa ipinanganak kaya sa DFA Office ang filling mo ng late registration.

REQUIREMENTS FOR DELAYED REGISTRATION OF REPORT OF BIRTH

Bring ORIGINALS and FOUR (4) SETS OFPHOTOCOPIES (Kindly arrange the copies according to the orderbelow)

  • REPORT OF BIRTH FORM (FA Form No. 40)
    Note:
    1. Four (4) forms should be ORIGINALLY FILLED OUT
    2. Entries must be TYPEWRITTEN OR PRINT LEGIBLY
    3. Entries must be at the time of birth of the child/subject
    4. For applicants born within the jurisdiction of Chicago, New York, Washington DC, formshould be notarized by the notary public
  • NEGATIVE CERTIFICATION OF BIRTH RECORD from the National Statistics Office (NSO) (CRS Form No.1

Question:

NSO tanong ko lang po ongoing po yong annulment ko sa 1st wife ko pero nagfile ng counter at nakakuha ng 2 records ng MC sa NSO pero yung sa 2nd wife ko nakalagay sa religion ko islam at siya naman ay catholic (same sa pilipinas ikinasal) pag nagfile ba ng bigamy yung 1st wife ko meron ba akong laban sa bigamy case? At pwede niya ba ako makasuhan ng bigamy since nasa broad ako?

Answer:

You might face legal problems because your annulment is not final yet.

Kahit nasa abroad ka maaaring makaka-suhan ka pa rin ng Bigamy dahil nagpakasal ka sa iba kahit kasal ka pa sa una mong asawa.

Nang kinasal ka sa una hindi ka pa naman Muslim, hindi pa nawawalang bisa ang kasal mo na iyon.

Question:

NSO ang problema ko kumuha ako ng CENOMAR ngayon nagulat po ako kasi may lumabas na record sa NSO na kasal daw ako sa isang hapon at ang kasal po ginanap sa Japan. Never akong lumabas ng bansa.

Ano gagawin ko para maayos ito?

Answer:

NSO records are official documents received from and filed by the Local Civil Registrar.

Hindi ka ba kinasal talaga? Baka naman nagpakasal ka sa ibang bansa na akala mo ay hindi ito mai-rerehistro dahil sa ibang bansa ang kasal mo. Hindi kasi pwedeng mag-imbento lang ang NSO ng kasal ng isang tao, kung ano ang nasa record nila, iyon lang din ang nilalabas nila sa taong nagre-request.

Subukan mong tingnan ang Advisory Of Marriage na na-receive mo, baka merong ibang information diyan na hindi sa iyo.

Magandang araw NSO, may tanong lang ako. Yung birth certificate ko po kasi tama naman po halos lahat… Kaso po pagdating po dun sa dulo na part… ung may mga box… Imbis po na JAYSON ang nakalagay (name ko) NELSON po nakalagay (name ng tatay ko)… Mahalaga po ba un? Or makaka-apekto kapag kumuha ako ng passport? Kung ipapaayos ko po yun.. Saan po at ano ang mga requirements? Salamat po.

Answer:

You can file for correction of entry at the Local Civil Registrar.

Kapag nagfile ka ng CORRECTION OF ENTRY sa Local Civil Registry Office (LCRO) kung saan naka-rehistro ang birth certificate mo, makikita naman ng LCRO ang difference at maayos nila ito sa loob ng ilang linggo o buwan.

Magandang araw po, pwede ko bang maipaayos yung birth certificate ko kasi meron pong JR. Pero gamit kong apelyido sa mother ko. Bale middle name ko yung JR.

Answer:

Pwede naman na ayusin ito sa munisipyo kung saan ka naka-rehistro, dun aayusin ang NSO birth certificate mo na may maling information. Dalhin mo lang yung NSO record mo na may mali and documents na nagpapatunay na hindi JR ang middle name mo, like baptismal and school records mo, then the rest of the requirements needed, munisipyo na ang mag advice sa iyo kung ano pa ang mga kailangan.
